Safety Tip: QRE Reports of Harm- Better Reporting
The actionability of QRE reports can be improved by describing:
- the specific harm
- resolution of the harm
- pharmacy's role in mitigating increased or prolonged harm
- all suspected contributing factors
Does your pharmacy:
- have an immediate response plan (clinical and communications) to a new harm event?
- review QREs to identify if appropriate actions were taken when the event was recognized?
